Golfers Gym System Requisition Form

Thank you for your interest in implementing the Golfers Gym Health and Wellness System in your school. This program is part of our $1 a Day initiative, designed to make health education and physical activity accessible and affordable for schools and students nationwide.

All participating applicants are required to:

Uphold the “Pass It On” system, ensuring that the benefits of the Golfers Gym Program are shared and extended to other schools or school districts in your network.

Submit registration fees at least 30 days prior to your implementation start date. This is necessary to allow proper customization and setup of the Golfers Gym experience in your school.

Unauthorized use is strictly prohibited and may result in legal action.
